REQUIRED DOCUMENTATION FOR 2022-2023 ONLINE PRE-K REGISTRATION

Before you begin, please not that you must upload the following documents while completing the application. NO applications will be accepted unless ALL documentation has been uploaded. ONLY COMPLETE ONE APPLICATION PER CHILD.
STUDENT INFORMATION

• Child’s Birth Certificate

• Child’s Social Security Card

• Child’s Current Immunization Record with seal or Immunization Waiver

PARENT INFORMATION

• Louisiana Driver’s License or State ID for both parents living in the home

PROOF OF RESIDENCY (must have 2 of the items listed below in the parent’s name and physical address on the document)

• Electric bill

• Water bill

• Cable bill

• Landline phone bill

• Wireless phone bill

• Tax Assessor’s bill

• Original or current Homestead Exemption

• Lease Agreement or Mortgage Statement on company letterhead or notarized

• Notarized Affidavit of Residency along with the homeowner’s picture ID & 2 proofs of residency

PROOF OF INCOME

• 2 consecutive current paycheck stubs from every adult contributing to income

• SNAP/Food Stamp Documentation

• Monetary Determination Letter from the Workforce Commission

• Social Security Administration Statement

• Current Foster Care Place Agreement from DCFS
